The post involves offering support and assistance to people with community care needs in order to facilitate discharge from hospital. Substantial experience of working within a care environment and HNC in Social Care would be preferred.
You must have the ability to be mobile across the Falkirk Council area.
You will work 37 hours per week.
This post is temporary until March 2026 due to service requirement.
If you are the successful candidate, you will be required to gain/maintain PVG scheme membership.
