To participate in the activities of an Integrated Community Care Area Team receiving requests for and under-taking holistic assessments of unpaid carers needs and commissioning packages of care working across professional boundaries when required in line with the Carers Act
Undertake carer assessment for both adult and young carers and where required by Team Manager Community Care assessments or on receipt of referral from other professionals.
Design and organise suitable packages of support having undertaken financial assessments where applicable, ensuring that adequate resources are available in conjunction with carers, the cared for and statutory and independent sector agencies.
Liaise with other local authority services i.e. old age psychiatry, learning disability, mental health, partner agencies, providers and the Carer Support Service to seek to meet needs in the most efficient and appropriate way for both the carer and the cared for.
Monitor and review unpaid carer needs and provision on a regular basis in liaison with the Carer Support Service.
Provide direct service and support to carers.
Collaborate with colleagues within the Health and Social Care Teams.
Participate with Team Manager in supervision, staff development and ERDP.
Participate in office duty system if required.
Maintain case records, both electronic and manual, provide information for performance indicators, unfulfilled procedures and statutory duties in line with national and local policy practices and procedures.
Health and Safety
Experience of working in a Health/ Social Care setting.
Demonstrable understanding of main issues facing Community Care.
Relevant professional qualification in Social Work.
SSSC Registered.
Effective communication skills (written and oral).
Ability to manage change.
Effective record keeping skills.
Ability to work to deadlines and under pressure.
Ability to manage own workload.
Competent use of IT equipment, software and good keyboards skills.
Assessment and organisational skills.
Finance skills.
Ability to negotiate and liaise in relation to care management.
You will be expected to travel efficiently and effectively between various work locations within Moray to meet the operational requirements of the Service. Due to the rural nature of Moray this is normally undertaken by use of a car.

Starting Salary: £43,204.20
£22.92 per hour
36.25 hours per week
