Ardent Social is proud to partner with Cardiff Council to recruit an experienced Social Worker for their Older People’s Team. This role focuses on supporting individuals living with dementia within residential and nursing placements, working collaboratively with providers and professionals to enhance care quality and opportunities for those in placements.
This is an exciting opportunity for a compassionate and experienced professional to make a meaningful impact on the lives of older adults in Cardiff.
Key Responsibilities:
* Conduct scheduled reviews and monitoring visits to residential and nursing homes to assess care quality and individual outcomes.
* Promote and enhance the lives of individuals living with dementia by working closely with placement providers to create better opportunities and improve care.
* Collaborate with care providers to address challenges and implement improvements in dementia care practices.
* Prepare detailed reports and provide updates to the Team Manager on placement visits, assessments, and outcomes.
* Liaise with professionals, including healthcare colleagues, to ensure seamless, multi-disciplinary support.
* Uphold safeguarding standards and adhere to statutory requirements in all care reviews and interventions.
What We’re Looking For:
* Qualified Social Worker with current Social Work Wales registration.
* Experience working in older people’s services, particularly with individuals living with dementia.
* Knowledge of residential and nursing care standards and relevant legislation.
* Strong communication and collaboration skills to work effectively with care providers and multi-agency teams.
* A compassionate and proactive approach to improving the lives of older people in care.

* Duration: Ongoing, with regular extension reviews.
