To participate in the activities of an Integrated Community Learning Disability Team receiving requests for and under-taking holistic assessments of client’s needs and commissioning packages of care working across professional boundaries when required.
Co-ordinate the assessment of individual needs and prepare care plans to meet said needs
Secure the resources or services required in responding to the identified needs
Develop relationships with service providers/other professionals
– Experience of working in a Health/Social Care setting
– Demonstrable understanding of main issues facing Community Care
– Relevant professional qualification in Social Work
– SSSC Registered
– Effective communication skills (written and oral)
– Ability to manage change
– Effective record keeping skills
– Ability to work to deadlines and under pressure
– Self confidence
– Self motivated
– Ability to manage own workload
– Competent use of IT equipment, software and good keyboard skills
– Effective assessment and organisational skills
– Finance skills
– Ability to negotiate and liaise in relation to care management
– You will be expected to travel efficiently and effectively between various work locations within Moray to meet the operational requirements of the Service. Due to the rural nature of Moray this is normally undertaken by use of a car/van.

Starting Salary : £43,204.20 to £47,370.05
