Hospital Discharge in Renfrewshire HSCP is recognised nationally as a leader in supporting the discharge of patients from hospital to homely settings without unnecessary delay. The service are seeking to appoint a Service Delivery Officer to support the process of efficient patient flow and discharge planning.
As a member of Renfrewshire’s Health and Care Partnership, you will provide leadership and management of staff in involved in discharge planning, to ensure the maximisation and optimisation of patient flow and service delivery, contributing to the overall management of the hospital discharge service. You will achieve this by ensuring effective deployment of all staffing resources and that both social work and NHS electronic systems are updated appropriately.
The role requires you to ensure discharge planning and early assessment is delivered to a high standard and promotes independence for patients/clients, ensuring rights, preferences and wellbeing are respected. This will include providing information about care services and coordinating arrangements for care homes, care at home services, housing related support, transport and meals deliveries.
You will work with and support senior managers, develop and implement workforce plans for the hospital discharge service, including effectively managing the recruitment, induction and ongoing training needs of the service, providing the opportunity for all team members to develop and deliver high personal performance on an ongoing basis requiring experience of supervising, supporting and managing the workload of staff aswell as managing staff absence, budget management, risk assessment and health and safety.
You will ensure effective monitoring of services is in place and managed, with a view to responding quickly and sensitively to changing demands within the community, ensuring priorities are met effectively and key performance targets are achieved i.e. delayed discharges
The role requires you to manage daily discharge planning and early assessment activity through staff allocation and use of electronic systems. You will be expected to record and monitor work using electronic case recording systems including Eclipse, therefore must have IT, computer and keyboard skills including Microsoft Office Applications.
You will be supported by RHSCP colleagues working across locality services, care at home services, care home services and community health. You will develop strong working relationships with acute hospital staff to plan discharges and increase knowledge and awareness of available community supports.
The team is located at the Royal Alexandra Hospital and operate in accordance with the Council’s WORKSMART arrangements which promote mobile and flexible work practices. The team are equipped as mobile workers and can operate throughout the Renfrewshire area from any Council premises. You will therefore be expected to travel throughout the Council area.
You must hold a Degree Level Qualification in Health and Social Care or Management or other relevant subject.
Proven experience of managing within demanding and complex services, updating, managing, and maintaining data through use of electronic systems, experience and understanding of financial and budget management and a proven track record of working in partnership with key stakeholders is essential.
Effective communication skills with the ability to communicate with staff, colleagues, managers, clients, service users in writing/emails and verbally and effective literacy and numeracy skills in order to record, interpret and report on information accurately is also essential.
You must have the ability to work well under pressure and experience of working effectively as part of a team within a demanding environment.
