Key Responsibilities:
Manage a caseload of children and young people in care, ensuring their needs are met in line with legal and statutory requirements.
Develop, implement, and regularly review care plans, pathway plans, and personal education plans to support the best possible outcomes for looked-after children.
Conduct statutory visits, risk assessments, and permanency planning to ensure children are placed in stable, nurturing environments.
Work closely with foster carers, residential care providers, schools, health professionals, and other agencies to support children's emotional, educational, and social development.
Prepare and present reports for court proceedings, care reviews, and other legal processes related to looked-after children.
Advocate for children's needs and rights, ensuring their voices are heard in decision-making.
Support children and young people in transitioning out of care to independent living or reunification with family where appropriate.
Maintain accurate and up-to-date records in compliance with statutory guidelines
