Rate: up to £32.65

Eden Brown are recruiting on behalf of South Gloucestershire County Council who are looking for a qualified Adults Social Worker to join their Home to Decide Team. This is a full time, locum role to start as soon as possible.

Supporting discharge from our acute hospital and community team we require a social worker who is able to complete fast paced work.
Complexity in hoarding, mental capacity assessments and supporting long term care needs.

Working in a MDT with OTs and social care practitioners.

– Initial assessments in persons own home
– Review and monitor working towards goals
– Identify if there are long term care needs
– Use of digital system ie mosaic.
– Predominately based in the office and within South Gloucestershire County. Opportunity to work from home.
