Guidance: Transition between mental health inpatient and community settings

NICE has published a new guidance on inpatient transition between hospital and community settings.

‘Transition between inpatient mental health settings and community or care home settings looks into the period before, during and after a person is admitted to, and discharged from, a mental health hospital. It aims to help people who use mental health services, and their families and carers, to have a better experience of transition by improving the way it’s planned and carried out. 

The guideline describes how people with mental health problems should be more involved in decisions about their care and sets out how people can be well supported before, during and after their time in hospital, so they can have the best chance of recovery.
 
The guideline is for providers, commissioners, managers and practitioners working in inpatient and community mental health and social care services. It will also be of interest to people who use inpatient and community mental health services, their families and carers.
