Report: Extended Child Practice Review – Elsie Scully-Hicks

The Cardiff and The Vale Regional Safeguarding Board (CVRSCB) has published an extended Child Practice review into the circumstances around the tragic death of Elsie Scully-Hicks.

The Review was commissioned by CVRSCB on the recommendation of the Child and Adult Practice Review Subgroup in accordance with the Social Services and Well-being (Wales) Act 2014 Part 7, Volume 2 Child Practice Reviews guidance.

The review has concluded that there was no information during the assessment of the adopted parents that could or would have predicted what happened to Elsie. Indeed, all the professionals involved in Elsie’s care reported a committed, motivated and caring family.

The review has found that assessment of the adoptive parents was strong and robust and was compliant with good practice standards.

However the review also found that there were areas of learning that should be acted upon to improve some systems and practices, particularly around timely sharing of information and data recording.

It also identifies occasions when there were missed opportunities to raise safeguarding concerns.
