Pilot scheme sets out home care priority
In a joint initiative between Abertawe Bro Morgannwg University Health Board and Swansea Council’s social services department, a new pilot project aims to ensure more people can receive vital care at home.
The Bay Health Care Team will see health and social care staff from both organisations working together to provide care and support to Gower residents.
Mark Child, Swansea Council’s cabinet member for wellbeing, said: “Many people prefer to remain living in their own home, even when they are frail or in poor health. The integrated care service aims to provide all the health and care support older people need to do this.
“This pilot proposal reflects the way our adult services team is pushing towards the transformation of social services for adults to provide sustainable, flexible services that deliver the right kind of support to help people remain independent at home until later in life.”
Most people will be referred to the Bay Health Care Team through their social worker, GP or hospital staff.
