‘Transforming Your Care’ plan for Northern Ireland outlined
Health Minister Edwin Poots has laid out his vision for health and social care in Northern Ireland at a seminar in Belfast.
The Minister said that from the beginning of his tenure he aimed for an “effective strategy to deliver a vision to drive up the quality of care, improve outcomes and enhance the patient experience.”
Speaking at the Efficiency in Health Care Services – Implementing ‘TYC’ (Transforming Your Care) Innovation and Cross-Border Collaboration, the Minister said: “We are on a three to five year journey with the emphasis on getting it right and in a safe manner. We are now making significant progress in implementing that transformation to deliver the right care, at the right time, in the right place.”
Minister Poots briefed the audience on Integrated Care Partnerships (ICPs), which bring together health and social care providers from both statutory and non-statutory sectors as well as service users and carer representatives to share their expertise, best practice and knowledge.
Mr Poots added: “We know that by ensuring that all parts of the system are working closer together we can prevent unnecessary hospital visits and admissions and provide the joined-up care and support which people need.”
