Dorset councils get £750k to transform health and social care services

Councils in Dorset are to get a £750,000 boost to help transform the way health and social care services are delivered.

Dorset County Council, Bournemouth and Poole will receive the government grant for key projects including early intervention and prevention, helping people to stay independent for longer and receive care and support in their homes or closer to their homes.

Work will also focus on delaying the need for long-term care and unnecessary hospital admissions.

There are also plans to extend the shared information and advice service open to everyone, shared signposting and support, shared services in hospitals including more help in arranging services for people who fund their own care.

The councils will be working in partnership with local NHS organisations on the initiative.
