SPCC was launched in April 2003 to improve patient’s access to a healthcare professional and to improve the outcomes for people with diabetes. The Collaborative programme aimed to develop skills and knowledge within General Practice to deliver rapid, sustainable and systematic quality improvements in the care provided to patients.
