Service Care Solutions are seeking a dedicated and compassionate Social Care Assessor to join an Adult Services & Health team at Liverpool City Council. This is a vital role, supporting individuals to access the services and care they need to live independently and with dignity. You will be part of a professional team that is focused on delivering high-quality services to adults in our community.
£22.00 LTD per hour inclusive of holiday pay
35 hours per week
3 month initial contract with possibility of extension after this Responsibilities
Conduct assessments to identify individual care needs and ensure appropriate support is provided.
Work collaboratively with service users, families, and multi-agency teams to develop care plans.
Monitor and review care plans to ensure they remain effective and responsive to changing needs.
Provide information, advice, and guidance on available services and resources.
Maintain accurate and timely records in accordance with organisational policies and procedures.
Ensure compliance with relevant legislation and Liverpool City Council standards.
Requirements
Previous experience in a social care or support role is essential.
Excellent communication and interpersonal skills, with the ability to build trust and rapport with service users and stakeholders.
Strong organisational skills and the ability to manage a varied workload effectively.
Knowledge of relevant social care legislation and safeguarding principles.
