Opus People solutions on behalf of Walsall Council have an exciting opportunity for an Social Worker to join thier ICS review Team.
This role will be 37 hours a week.
In this role you will:
To work in a MDT team, to complete initial and subsequent reviews of packages of care at home and short term placements put in place for people following discharge from hospital.
To identify if any OT therapy is needed, to promote independence through providing assistive technology and signposting to community assets and to determine if any, long term support is needed.
To complete revised support plans if there are changes to care, to complete MCAs and risk assessments when required.
To refer to extra care housing and in house provider services if required.
