Field Care Supervisor £ 23,000 – £26,500 per annum Central London
PURPOSE OF THE POSITION
Responsible for the efficient running of an area and management/ supervision of Care/Support Workers and care provided to Service Users within that area in accordance with systems, policies and procedures, Local Authority policies and Care Quality Commission (CQC) standard. Implementing Service User needs assessments, care/support plans, risk assessments and reviews. Supervising approx 35 carers.
This role will require to step in as a coordinator when necessary assisting Home Care Workers promptly and conducting all business in a friendly, courteous and knowledgeable manner.
You will carry out on call duties as scheduled for each week.
PRINCIPAL DUTIES AND RESPONSIBILITIES
Key Responsibilities
* Responsible to carry out Service User needs assessments, care/support plans and risk assessments.
* Take lead and attend the Service User’s first call to introduce the Care/Support Worker to the Service User.
* To be responsible for a defined patch of the clients and building good working relationship with social workers and others health care professional
* To compile client information/carer management actions and report weekly by utilising Key Performance Indictor (KPI forms to monitor the performance and compliance against each patch).
* Ensure the standards and targets agreed for a weekly deadline are met.
* To organise and attend Service User reviews, including reviewing and updating of needs assessment, care/support plan and risk assessment documents
* To liaise closely with Care Co-ordinators to ensure correct Service User and Care/Support Worker matching for new Service Users.
* Deputise for Care Co-ordinators allocate unassigned calls during office hours as required.
* To allocate all unassigned calls during on-call hours – where visits can’t be assigned, undertake the visit
* To ensure that all changes to care schedules are reported, without delay, to the Care Co-ordinator and recorded appropriately.
* To undertake on-call duties as directed and record/report all on-call activity to the office.
* Undertake field-based training to ensure Care/Support Workers are working within their competency level.
* To work along with Quality and compliance Manager and ensure quality control procedures and processes are in place.
* To escalate all complaints and safeguarding investigations to the Quality and compliance Manager.
* To assess Care/Support Workers again Skills for Care Common Induction Standards during shadow working sessions.
* Ensure all filing related issues in your patch is completed upon risk assessment, face to face review, spot check. medication competency, and direct filed observation.
* Qualifications required :
* NVQ2
* Willing to work towards NVQ3
* Clean enhanced DBS
