Complex Care Case Manager (non-nursing)
£32,000 (negotiable) plus £1500 quarterly bonus
Liverpool/Northwest with Remote Working
Full-time, 35+ hours per week
We are seeking a motivated Complex Case Manager who is dedicated to positively impacting our clients and promoting a positive atmosphere within their care team. Responsibilities:
* Act as a role model, fostering a supportive environment to empower staff in delivering quality care.
* Uphold internal and external standards, adhering to CQC regulations, contractual requirements, NICE guidelines, company best practices, and Equal Opportunities.
* Take joint ownership of a caseload of complex clients and manage their care package holistically, in collaboration with their appointed Nurse Case Manager.
* Develop, review, and implement client care plans and risk assessments following CQC regulations and best practice guidance.
* Co-ordinate and lead the transition and ongoing continuity of all allocated complex care packages, working with clients, professionals, and Nurse Case Managers.
* Maintain high standards of documentation and record keeping in compliance with CQC, national legislation, and local standards.
* Conduct regular auditing of records and daily evaluation notes, reporting trends and taking appropriate actions.
* Inform the Service Manager of any issues that may impact the quality of care or the health and safety of clients and healthcare workers.
General:
* Comply with the Company's procedures regarding Conduct and Confidentiality.
* Take responsibility for personal health and safety, as well as the health and safety of others.
* Continuously develop professional skills through relevant training and updates.
* Ensure systems and databases are kept up to date.
* Complete any other tasks assigned by the Line Manager within the scope of the position.
