Are you an experienced Support Worker looking for the next step in your career? If so, join Elysium Healthcare as the Physical Health Assistant where you will be developing existing initiatives in physical health monitoring and interventions.
Working as part of the nursing physical health team within brain injury, this is a newly created, specialist role, where you will be combining pre-existing mental and physical health skills in order to promote health and wellbeing to our service users.
The aim of this role is to build and maintain positive working relationships between our services and the local GP’s and other external healthcare providers of all backgrounds. This will ensure that the people we care for and their physical health needs are addressed in a timely and expert way.
As a Physical Health Assistant, you will be:
Providing and undertake a wide range of physical health monitoring.
Supporting colleagues in undertaking physical health assessments.
Working collaboratively with primary health, develop and maintain links with GP practices, and other physical health departments.
Monitoring and managing PPE compliance and medical supplies
Monitoring and maintaining Physical Health Dashboards
Undertaking additional Training to support role – Phlebotomy, Assistant Practitioner, Factor Administration, and ECG’s etc.
Ensuring the people we support have access to relevant appointments and screening relevant to age and medical history.
Working in collaboration with the Registered Nurses to ensure the Hospital Passports and Heath Action Plans are kept up to date.
Maintaining up to date clinical records in line with electronic records system – Care Notes.
Supporting the Nursing team to monitor maintain medical emergency equipment at site.
Working in partnership with health care professionals and other agencies to reduce the impact of poor physical health on the client group, improving outcomes.
Promoting physical health initiatives to health care professionals and other agencies.
Participating and developing relevant local clinical audits and research, to inform service delivery and development.
To be successful in this role, you will have:
Proof of continual professional development
Assistant Practitioners course – willing to complete if successful
Advanced qualification and or experience in working in primary care settings
Current Driving License
Experience in setting up new Healthcare Initiative
Experience in brain injury
First aid trained
Good working knowledge and understand of physical health needs in people with brain injuries.
Experience of implementing evidence based practice.
Ability to cope under pressure and liaise and seek advice when managing complex situations.
Ability to reflect own performance.
Able to communicate at various levels with empathy – internally and external.
Experience of working independently
