Job Scope:
We are seeking a Care Coordinator to join the Leaf Complex Care team, a brand of Catalyst Care Group. As a Care Coordinator, you will play a vital role in delivering tailored support for autistic individuals and those with learning disabilities, helping them live fulfilling lives in their communities.
In this role, the Care Coordinator will manage all inbound inquiries, ensuring they are handled promptly to meet client needs. You will exceed client expectations by delivering high-quality service and building strong relationships with clients, clinicians, and colleagues.
The Care Coordinator is accountable for holding clinicians to the highest standards of care, maintaining clear communication, and ensuring compliance with internal and external support requirements. You will also oversee audits, complete risk assessments, and conduct site visits as needed.
This Care Coordinator position includes on-call duties one weekend per month and one weeknight per week. After the initial discovery phase, hybrid working options will be available, allowing for up to two days of remote work. At Leaf Complex Care, we emphasize person-centred, CQC-regulated care based on Positive Behaviour Support approaches.
Do you have?
* At least NVQ level 2 in Health and Social Care (desirable).
* Experience working in a care environment is required, with a preference for those who have supported individuals with Learning Disabilities, Autism, or ADHD.
* Previous experience dealing with or within a high-volume incoming call and query environment.
* Practice in using relevant business software such as Microsoft Word, Outlook, and Excel.
* Driver’s license
