We are establishing an innovative new service called Homefirst which will provide reablement support to individuals, supporting them to be discharged from hospital.
The team will work as part of an integrated hub with other existing intermediate care services, to support people living within the community, to maximise their independence and achieve their personal outcomes.
Services and support will be delivered by a multi professional team which includes Occupational Therapists, a Physiotherapist and a Social Worker.
We are looking for a qualified Social Worker who will have a key role in the delivery of this new service. The post holder will support the work of the intermediate care hub by undertaking a range of statutory tasks, including co-ordinating and undertaking single shared assessments, risk assessment and, where appropriate, Carers’ assessments.
This role will be expected to be able to provide advice and guidance, if necessary, should an adult be at risk of harm and in need of protection under the Adult Support and Protection (Scotland) Act 2007 or, Adults with Incapacity (Scotland) Act 2000.
The post holder will also undertake the initial statutory review required following the commencement of any new packages of care and support. Working as part of a multidisciplinary team and liaising closely with other members of the integrated intermediate hub, the post holder will support team members, adults and their families and carers, to balance the complex areas of risk and need with choice and rights.
The successful candidate will hold a degree or post graduate qualification in Social Work, which allows registration with SSSC as a Social Worker. You should also be registered with SSSCC, although applications from candidates currently in process of registration may be considered.
Applicants must be able to travel efficiently and effectively between various work locations within Orkney to meet the operational requirements of the Service.

Closing Date: Sunday 3 November 2024
