An exciting opportunity has now arisen for a Qualified Social Workers to join our Citywide Hospital Social Work Team; Glasgow HSCP are embarking on implementing the Discharge Without Delay National programme and will seek collaboration with acute colleagues to maximise early referral and intervention from Social Work Services where appropriate. You will be part of a team working across a range of professionals and organisations in acute and community settings.
Our focus is on person-centred and timely discharge via a variety of discharge pathways with our first choice being a return home where possible. This is a challenging, fast paced role with a number of service developments being implemented. We therefore are seeking energetic, talented, resilient and committed people to come and work with us.
You will have sound knowledge of assessment and care management; have experience in both ASP and AWI legislation; and work in a person centred manner which will be fully embedded in your practice. You will be confident in managing risk and looking for innovative ways to promote independence and link people into the local community. You will be committed to continuous professional development.
Applicants must possess a Degree / Diploma in Social Work (or Social Work equivalent).
Current SSSC registration as a Social Worker or registration within 6 months of taking up a position, is essential.
Membership of the Protection of Vulnerable Groups Scheme will be essential on taking up appointment along with SSSC Registration. Please note successful candidates for the above position will be required to pay for their PVG Membership/or Scheme Check Please note as part of the local government pay deal, local authority staff are currently not required to pay SSSC fees.
