This is an exciting opportunity to be part of a team putting people first to make sure they get the right support at the right time, ensuring the same day discharge for those individuals who are clinically ready to return home. As part of our new Home First team, you will work alongside Health & Social Care and Acute colleagues whose focus is prevention of delays in hospital discharge arrangements. Working alongside the individual and their families and other support networks ensuring good quality support in those first days after returning home. Within the North Lanarkshire Partnership our ambition is beyond same day hospital discharge, but prevention of admission at the front door of the hospitals and within community, where response to crisis situations can prevent hospital admission.
Working within this fast paced service, you will help us deliver a high level of customer service and community support to service users across North Lanarkshire. Working closely with the Service Delivery Co-ordinator, you will use a dynamic scheduling tool to ensure the people we support receive a prompt and efficient person centred support which promotes greater independence and choice.
To undertake this role, you will need a positive approach and be able to communicate effectively with a range of individuals including service users, their families and other partners/agencies. We are looking for someone with excellent organisation skills, is resourceful and works well under pressure. Experience working in a social work or home support related environment is essential. Possession of a SQV Level 3 or equivalent qualification is desirable however successful applicants who do not hold this qualification must demonstrate a willingness to work towards achieving this within a reasonable timescale.
Some posts will require evening and weekend working, full details will be discussed further at interview.
