Several exciting new opportunities for qualified Social Workers have has arisen within the Community Hospital Discharge Team. Working within a fully integrated Social Work and Health Multi Disciplinary team. You will complete holistic and high quality assessments to ensure that supportive interventions are provided to facilitate a timely discharge to adults of all ages. Individual’s support packages should be person centred and will ensure the best outcome for the person; as well as working within legal frameworks and HSCP policy and procedures.

This post is considered as Regulated Work with Children and Adults under the Protection of Vulnerable Groups (Scotland) Act 2007 and as such membership with the PVG scheme is essential. Further details of this scheme can be found here.
Preferred applicants will be required to become members of the relevant PVG scheme, or undergo a PVG Scheme update check prior to a formal offer being made by West Dunbartonshire Council.
