West Lothian Older People Services are recruiting for a Social Worker to support the work of the Older Peoples Practice Team. This team primarily covers all aspects of assessment and case management of those aged 65+ within West Lothian. As part of this team, you will be expected to make a positive difference in the community through undertaking needs led and person-centred assessments and ongoing management of complex care needs. Alongside meeting the statutory obligations of the post, your key responsibilities will revolve around partnership working and the provision of professional advice and guidance to individuals, families and groups within the community.

Candidates should have excellent verbal and written communication skills, completing all required reports, assessments and case notes to a high standard. In addition to this, candidates should promote anti-discriminatory practice and be able to engage with individuals from all age ranges and backgrounds.

This position requires registration with the SSSC. The successful candidate will have six months from commencement in this post to register with the SSSC. If you are already registered with SSSC you will be required to add West Lothian Council to your account
