As part of the West Lothian Health and Social Care Partnership (HSCP), the Team provides an assessment and care management service to adults under 65 with a Learning Disability and/or Autism.
The successful candidate will be based within a well-established and developing team. This is an interesting and challenging opportunity for an enthusiastic person to work with young adults with a range of complex needs who require support to maintain their independence and transition from Child to Adult Services. Candidates should ideally have experience of working with this client group.
The successful candidate will have an ability to work as part of a team, respond to demands of a busy service, be able to follow procedure, have excellent communication and report writing skills. These tasks will be carried out in partnership with service users, family members, Education, Health and other professionals and agencies.
The post requires a good understanding of self-directed support and outcome focused assessments. You will also undertake Duty work and Adult Support & Protection investigations in accordance with service standards.
A professional Social Work qualification is essential.
This position requires registration with the SSSC. The successful candidate will have six months from commencement in this post to register with the SSSC.
This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council.
