Working within an integrated Social Work & Health Multi-Disciplinary Learning Disabilities Locality Team, you will manage complex community care cases in respect of people with Learning Disabilities, their families and carers.  Your role will entail assessment, care management and intervention services within a multi-disciplinary framework to ensure best outcomes for service users and to work within the framework of relevant legislation and Council policies and procedures.

You must have relevant experience of Social Work practice and be able to demonstrate that you understand the legal basis and social policy context underpinning social work practice, social process and human development, and social work approaches and interventions.

Experience and understanding of adult protection and the assessment and care management process; effective analytical, IT, interpersonal, time management and communication skills (both verbal and written); the ability to work on your own initiative, prioritise your own workload, maintain case records, work in partnership with other agencies and demonstrate delivery of services through case work and crisis intervention is essential.

Applicants must have a BA (Hons) in Social Work or equivalent and be registered with SSSC.  Full training and support will be given to ensure that you are able to maintain the department’s commitment to continuous improvement.

Please note that this post is temporary, subject to business needs and funding for 16 Months or until the requirement for the post ceases, whichever date is the earlier.
