A permanent vacancy has arisen for a Social Worker in the SCOPE Team working with individuals with a learning disability and/or autism who have complex care requirements. The team will be supporting individuals from the age of 14 years and upwards at different life stages, from transition from school into adulthood, moving to their own accommodation and beyond into older age.
The team will be made up from disciplines from both Health and Social Care, such as Occupational Therapists, Psychologist, Carer Support Worker and Outreach Workers. While working with aligned professionals including Speech and Language Therapists, Community Learning Disability Nurses and Dieticians.
SCOPE is leading in developing locally based provision, which will improve the lives of those with complex care requirements as well as their families and carers. The team will have a whole person approach and will embed the principles of positive behavioural support into the delivery of support.
The successful applicant will assist in maintaining vulnerable people in the community by providing a range of social work and care management tasks in accordance with the New Care Standards and the SSSC Codes of Practice. These include carrying out Outcome Focussed Assessments, the negotiation of flexible packages of care, and the monitoring and reviewing of individual care packages delivered via Self Directed Support (SDS). Responsibilities will also include statutory work in accordance with the Adults with Incapacity (Scotland) 2000 Act and the Adult Support and Protection (Scotland) Act 2007.
You must have a DIPSW, CQSW, CSS and relevant Social Work Qualifications, a current registration with SSSC and be eligible to register with PVG scheme. You must have a valid UK driving licence as this post requires you to travel to a variety of locations across Perth and Kinross.  

The Individual

You will be motivated, engaging, and possess excellent interpersonal and communication skills. Effective time management is essential for a successful autonomous worker to manage a complex caseload with the ability to be flexible and respond to crisis/competing demands being integral to the role. An in-depth understanding of Self Directed Support and how this is delivered is essential.
The ability to integrate and cultivate successful links within the team, partnership as a whole and across key services is highly desirable. Knowledge of services, challenges, demands, and demographics within PKC would be an advantage. 
You must have experience and skills of client needs assessment and care package reviews. You will be required to demonstrate a sound knowledge and understanding of social work principles and values.
Experience of Adult Support and Protection Procedures and all relevant legislation such as Adults With Incapacity and Carer Act will be essential.
This post may be suitable for candidates who have recently completed their Social Work Degree but have not yet graduated. In this circumstance, a Social Work Assistant contract will be offered until you receive your qualifying certificate and are eligible to register with the SSSC as a Social Worker.
