Your new role
We are looking for a social worker who will be enthusiastic and creative, committed to a collaborative approach with individuals to promote well-being and to safeguard and promote independence, and who will focus on what matters to people who are seeking support. They believe that by exploring people’s goals and aspirations, their strengths, and networks of support, we can help people to stay independent for longer and connected to their communities.
Assessing and care managing the complex needs of older people and their carers to ensure their health, wellbeing and social inclusion. To liaise with Reablement and other professionals to ensure seamless care pathways which safeguard and promote independence.
To promote partnership working which supports the person-centred outcomes to be delivered for older people. To undertake safeguarding investigations as required.

What you’ll need to succeed
* Enhanced DBS Check
* Knowledge of Social Services and Well-being (Wales) Act 2014
* Appropriate social work qualification (i.e., Degree in Social Work, DipSW or equivalent).
* Registration as Social Worker with Social Care Wales
* Ability to organise, prioritise and manage a caseload
* Full Driver’s license & access to own travel
