Your new role
As a qualified social worker will be working within a strong established Multi-Disciplinary Community team, working as an integral part of an MDT support citizens recently discharged from hospital to regain independence, link to their community and determine if long term support is required. To support citizens to manage risk including section 42 enquiries. undertaking caseloads with Citizens and carers to identify needs and outcomes. This is working across sites in the city within the newly formed Early Intervention Community Teams. Hours of work are 35 Hours a week over 5 days Mon-Fri and will cover duty on rotation when attendance in office is required for full day.
In this position you will need to be able to work in a fast paced environment and have recently worked as a qualified Adults Social Worker with experience in keeping and maintaining accurate records. You will have excellent communication skills and feel comfortable to pick up new and existing cases. It is imperative that you have an in depth understanding of the legislation underpinning Social work and be able to manage any challenges with appropriate methods which will vary from case to case.
What you’ll need to succeed
Previous direct experience of delivering social work to Adult Service users is essential. You must also have knowledge of key legislation, able to produce reports and maintain records. To be successful you will require a Social Work qualification (BA Social Work, CQSW, DipSW, CSS or CCETSW approved equivalent). It is also essential that you have a current professional registration and have a minimum of 5 years’ experience working with adults in a Social Worker position. Knowledge of ‘CareFirst’ and the 3 conversations model desirable but not essential.
