Social Worker – Hospital, Assessment & Care Management Full Time

Job Description

A Social Worker is required to undertake Assessment & Care Management and other Social Work responsibilities including liaison with health, private and voluntary agencies. You will focus on complex discharges in a hospital setting and in urgently assessing those who are discharged home to await assessment.

Requirements

You will hold a Diploma in Social Work or equivalent along with Mental Health Officer Award. It is essential that you have an up to date knowledge of Community Care legislation and practice, are able to communicate clearly, have good interpersonal skills, experience of inter-agency work and hold a current driving licence.

This position requires registration with the SSSC. This post is considered Regulated Work with Vulnerable Children and Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. You will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by Comhairle nan Eilean Siar.

Salary for the above post is inclusive of Distant Islands Allowance. Appointment to the post will be on the first point of scale with annual incremental progression.
