Your new role
The role of the team is to ensure referrals are triaged and screened appropriately, signposted to other services, professionals and organisations where needed and that any urgent or crisis intervention is offered before moving cases through to the most appropriate team. To undertake assessments, care planning and specific Social Work tasks which are needs led and target resources to those who are in greatest need within the resources available. To undertake specialist social work care planning including the provision of direct specialist social work interventions. The ideal candidate would be experienced in assessment and care planning and will also need some experience and competence in carrying out capacity assessments, best interest processes and working generally within the Mental Capacity Act, alongside an understanding of Safeguarding of Vulnerable adults. You will need to be resilient and able to work under pressure in a Duty function, but equally be able to work as part of a team, supporting each other and planning work appropriately. Develop cost packages of care which are flexible, cost effective and responsive to the changing needs of the individual. Promote independence, self-determination and the maximum choice through access to a range of services provided by the independent and statutory sectors. To contribute to the management of the adult care budget and the maintenance of expenditure within set budget limits. Ensuring that individuals have the information made available to them in an appropriate format which enables them to make informed choices.
