We need social workers to fill temporary positions in the Locality Hub teams to support hospital discharge
These posts will be for 4 months
The main role of the workers in the NW will be to support hospital discharge following a home first philosophy to ensure that people get the opportunity to make big decisions in heir own homes where ever possible and live their lives as independently as possible. You will work in a multidisciplinary team with colleagues from Occupational therapy Physiotherapy ,home care and medical staff . This is an opportunity to gain experience in working in acute and rehab based services.
You will need a current recognised social work qualification and be registered with the SSSC.
You will be responsible for the management of a caseload people who have support needs , you will assess and review cases to ensure outcomes are met.
There will be a proportion of Adult protection work including Duty to Inquire investigations and follow up from this.
You will have regular supervision and support and be part of a team with good peer support.
We are looking for people with experience although these posts may suit newly qualified workers who have some statutory placement experience.
You should understand about relevant legislation including Adult Support and Protection, Self Directed support and Adults with incapacity.
This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by the City of Edinburgh Council.
Our salary range typically reflects the initial starting salary and annually increases until it reaches the top of the range. For roles in grade 5 or above, annual increments in our contribution zones are dependent on achieving agreed performance goals.
