Social Worker – Continued Health Care (CHC) Full Time

Your new role
Birmingham City Council are looking for 15 dedicated and highly motivated Social Workers to join the Continued Health Care Teams: You will be dispersed around the Birmingham Constituency teams, 1 in each with the remaining 5 being directed to the ones with the most need.

You will be required to undertake assessment/review with citizens being discharged from hospital, with the reviews will be carried out jointly with CCG colleagues to determine health and social care needs and the aim will be to agree an appropriate funding stream decision – local authority, CHC, or joint funding split. Experience of working knowledge of Care Act, MCA, MHA, and CHC framework and have experience in, and confidence to, advocate for the person and LA in MDT decision making forums while maintaining positive working relationships with other professionals

We are offering rolling 3 month contract placement dependent on throughput of work. We are seeking someone who is ideally Carefirst ready, and very experienced social work background, and able to undertake managing a complex caseload. We are also looking for someone who can commence working with us quite quickly. Of utmost importance we are looking for someone with excellent safeguarding experience.

What you’ll need to succeed
To be successful in this position you need to be post 2 years qualified Social Worker with previous CHC experience. Have a passion for improving the lives of Birmingham Citizens and supporting those in need. You must be registered with SOCIAL WORK ENGLAND and hold a Social Work degree (BA Social Work, CQSW, DipSW, CSS or CCETSW Have in depth working knowledge of Care Act, MCA, HRA, MHA, CHC essential. Required excellent competency with IT skills. Able to work autonomously
