We are looking to appoint a Social Worker within West Dunbartonshire , to work as part of the multi disciplinary Community Older Peoples’ Team within the Integrated Health and Social Care Partnership. The post holder will provide an assessment and care management service to individuals over the age of 65 and their carer’s, to be carried out within the framework of legislation, relevant policies and procedures. They will provide and review individual packages of care and promote social inclusion and health andwell-being of individuals. The successful candidate will have a strong sense of professional identity and an ability to work bothindependently and as part of an integrated team.

This post is considered as Regulated Work with Adults under the Protection of Vulnerable Groups (Scotland) Act 2007 and as such membership with the PVG scheme is essential. Further details of this scheme can be found here.
Preferred applicants will be required to become members of the relevant PVG scheme, or undergo a PVG Scheme update check prior to a formal offer being made by West Dunbartonshire Council.
