An exciting Recruitment Opportunity has arisen for the position of Social Care Worker within Care at Home Services in Glasgow.
Reporting directly to the Team Leader, you will be responsible for undertaking initial assessments for care planning, and review of existing service users. To do this you will work in partnership with Occupational Therapists, Area Operations Managers and local internal and external agencies to achieve the best outcomes for service users.
You will be required to ensure delivery meets National Care Standards and Glasgow City HSCP service objectives whilst ensuring performance management targets are achieved in relation to reviews and assessments.
To succeed in this role, you will be a confident, self-motivated, and driven individual with a flexible approach, together with the ability to put forward new ideas and solutions that add value to the Care at Home Service.
The successful applicant will demonstrate excellent interpersonal skills, an ability to communicate effectively with service users and their families, navigate and record on Care First and work collaboratively with internal/external agencies.
There will also be a requirement to participate in a Duty Rota for Care at Home.
Applicants must be experienced in assessment and care management.
This position is subject to membership of the Protection of Vulnerable Groups (PVG) Scheme.
