As Social Care Practitioner you will work as part of a team on a shift rota basis (including weekends)
You will act as “key worker” and actively contribute to the care planning process for a number of identified key residents, including risk assessments, reviews and medication management.
You will ensure that care practice is non- discriminatory and that residents personal beliefs and preferences are appropriately acknowledged
You should be aware of and responsible to the needs of older people with mental health problems, e.g. dementia
You will provide for the personal care, support and emotional needs of older people requiring residential care.
You will provide a service compatible with professional social work practice and within the policies, procedures and standards set by the service.
It is desirable that you hold an SVQ 3 Health and Social Care (or equivalent) – if not you should be able to work towards this. You should have experience of working with older people.
The skills and experience you must have:
Must hold or be willing to achieve Social Services and Healthcare SCQF level 6 (equivalent to SVQ2) or equivalent, to meet the Scottish Social Services Council (SSSC) registration for the post.
This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by the West Lothian Council
This position requires registration with the SSSC. The successful candidate will have six months from commencement in this post to register with the SSSC. If you are already registered with SSSC you will be required to add West Lothian Council to your account.
