You will assist the Joint Dementia Initiative Team in providing services to older adults who have a diagnosis of dementia to live in their own home for as long as possible with the best quality of life.

You will provide practical support and supervision within their home and within a wider community setting.

You will be involved in assessments, maintaining accurate records and reports on individuals and co ordinating groups activities. Rotas, communication with Social Work, family, and carers.

Including working with Older Adults Team Locality Social Work teams.

You should have knowledge and experience of working in a community care setting and or direct experience of working with people with Dementia.

The ability to work jointly with other disciplines is required. You must be motivated to support people to lead full and positive lives by operating in a person-centred capacity.

HNC /SVQ3 in social care or equivalent would be preferred.

You must have the ability to be mobile across the Falkirk Council area.

If you are the successful candidate, you will be required to gain/maintain PVG scheme membership.
