This is an exciting secondment or fixed term opportunity based within the Community Addiction Recovery Service(CAReS) in South Lanarkshire. The ideal candidate is a Social Worker who is looking for a new challenge and is experienced in the co-ordination and management of people who have multiple complex needs.
This post is an 18-month Test of Change externally funded by monies invested by CORRA Foundation in response to continuing national rise in drug related deaths. It aims to work with collaborative partners to implement a quality improvement test of change capable of delivering a robust “no wrong door” multi-professional care pathway.
We expect that the pathway will be used to support people who have multiple risks arising from underlying physical, mental and social compromise. They are likely to be people who do not meet threshold criteria for ASP intervention and/or used as tool to prevent escalation to ASP referral and formal management.
The successful candidate will work with colleagues across H&SCP but will primarily be linking with CAReS Team Leaders. The people who are referred to this process will also be linked with a lived experience Peer Support Worker who will complement and build connections between all involved professionals, the individual and any identified natural supports (family and/or friends).
