Job Description

In partnership with community planning partners, voluntary and independent provider organisations, we offer a range of services designed to enable, support, improve and protect the health and social care of people using our services.

2 posts – 37 hours per week / 30 hours per week

The successful applicant will have experience in caring for older people, preferably within a residential setting and be familiar with all aspects of personal care.

You will be able to construct a person centred support plan, implement, review and evaluate its effectiveness for the service users for whom you have responsibility. You will contribute to the assessment and review process and administer medicines as prescribed.

Shift and weekend working is essential.

The successful candidate is required to hold a SVQ Level 3 in Social Care or a HNC or be willing to work towards these qualifications.

Responsibilities

• To undertake supervision of specific tasks as a member of a care team to enable service users to maintain and develop their individual lifestyles.
• To contribute as a key worker in consultation with senior staff, colleagues, service users and their families and relevant others to the formulation of a care plan for each individual.
• To co-ordinate, in consultation with senior staff, to the assessment and review of individualised care programmes.
• To provide and ensure a high level of support to individual service users as determined by their care plan to meet their physical, emotional, psychological and social needs.
• To undertake, as directed, such physical care duties in or outwith the unit, as are required to maintain the well being of service users including aids to daily living.
• To work under supervision with families (carers) and other departmental or agency staff or relevant others in promoting the care of individual and groups of service users.
• Supervise service users or assist with the administration of prescribed medications to the required dosage.
• Assist service users with dementia to attain personal well being, applying own taught accredited learning to this process.
