This is an exciting opportunity to join West Dunbartonshire Health and Social Care Partnership’s Community Hospital Discharge Team. Community Health and Care Services within the HSCP are fully integrated. The Community Hospital Discharge Team consists of social work and health staff.
The team offers services to adults, older people and their carers which aims to maximise
individual’s health and well-being and promote safety and independence through a co-ordinated multi- disciplinary / agency approach.
The integrated team works in line with the HSCP’s objectives to support individuals to remain/reside at home or in a homely setting. The post holder will manage and co-ordinate the delivery and development of Integrated Health and Social Work services with other service leads and integrated operations manager. Substantial post qualifying experience as a Social Worker is essential along with a robust knowledge of current relevant legislation.
This post is considered as Regulated Work with Adults under the Protection of Vulnerable Groups (Scotland) Act 2007 and as such membership with the PVG scheme is essential. Further details of this scheme can be found here.
Preferred applicants will be required to become members of the relevant PVG scheme, or undergo a PVG Scheme update check prior to a formal offer being made by West Dunbartonshire Council.
