Job Details

The post is for a Senior Practitioner-Transitions in a specialist Children with Disabilities team within the Children and Families Service. It is an exciting time to join the team in this newly created post, which will focus on work with young people approaching their school leaving age and preparing to move on into Adulthood. The successful candidate will work as part of a team, supporting young people and their families to access services specific to their needs.

The successful candidate will:

• Have a sound understanding of and experience in supporting young people through the transition process.
• Have a sound understanding of Self-Directed Support and experience in supporting families to access this.
• Have knowledge and understanding of Child and Adult Protection guidelines and information sharing protocols
• Be required to and have experience in undertaking professional supervision of Social Workers and Social Work Assistants supporting their professional development and ensuring Council policy and procedures are followed.
• Be required to work within service processes and procedures to assess and undertake appropriate and relevant planning for children to ensure positive outcomes are completed within timescales.
• Complete Quality Assurance audits
• Undertake thorough and informed comprehensive assessments of need and risk.
• Work individually and/or collaboratively to produce plans to support children, young people and their families.
• Assess, manage, monitor and reduce risks to service users and others.
• Contribute to complex assessments of need and risk with an aim of improving outcomes whenever possible.
• Carry a caseload of transitions cases with varying complexity, requiring specialist skills and knowledge to enable the best possible experience of transition from children and families services to adults services for young people.
• Provide written reports as required, including informed recommendations concerning care/action plans.
• Attend meetings and case reviews.
• Update and ensure the accuracy of all relevant service user records and taking responsibility for the effective application of information management systems.
• Engage and work in partnership with adult social work, service users, family members, Police, Health, statutory/voluntary agencies and others as appropriate.
• Chair meetings and review panels as appropriate.

The Person

A Diploma in Social Work (or equivalent) qualification is essential, as is registration with the Scottish Social Services Council.

You will have one of the following:

An accredited post-qualifying award (e.g. Certificate in Child Protection Studies, Mental Health Officer Accreditation, Practice Teacher Award, etc.)

You are required to register with Scottish Social Services Council (SSSC) within 6 months of taking up employment in this role and to complete the required Post Registration Training and Learning (PRTL) over the first registration period.

You will save money on your SSSC fees as these will now be paid for all roles and grades by the Scottish Government.

Before confirming your appointment, you will be required to obtain Protection of Vulnerable Groups (PVG) scheme membership through Disclosure Scotland and become a member of the relevant PVG scheme. 

We are currently shaping our future workstyles however, in light of current advice, we are introducing blended home/office working that will include office access days each week with homeworking for the remainder of your working time.
