Senior Practitioner Adult Hospital Discharge Team

Your new company
You will be working for a local authority in the south coast seeking to add experienced staff within adult social care to enhance the good work the service is providing to the hospital discharge team.

Your new role
You will be working within a hospital discharge team to focus om timely discharges some in relation to covid19.. You will be supporting a team of social workers and working with complex cases.

What you’ll need to succeed
You will need a SWE registration and previous experience as a senior practitioner in adults services, ideally within HDT.

What you’ll get in return
You will earn a competitive rate of up to £43 per hour dependent on experience and the opportunity to make a real difference.
