Job Description

What is the role?

Under the direction of a Senior Manager, the post holder will co-ordinate the implementation of Self-Directed Support (SDS) in line with both local and national legislation. They will ensure systems, policies and operational mechanisms are in place to support the effective delivery of Self-Directed Support.  The post holder will be required to influence and embed Self-Directed Support across the HSCP to ensure it can deliver individual service users and carers with options over the care and support they receive.

The Grade for this post is Grade 4 / Level 12, the hourly rate is £25.37 to £26.92 and the annual salary is £46,300 – £49,129.

The successful candidate will be required to work flexibly between home and the office and the frequency for attending the office may vary depending on the exigencies of the service.
