Qualified Social Worker (Adults) Full Time

5x qualified social workers with at least 2 years experience, to work across the Sandwell community

We are looking for 5x Social Workers to work across Adults Social Care, who will work across the 6 community social work teams, and must be able to hit the ground running. You must have at least 2 years experience since becoming qualified.

Hours 37 hours per week and required to work the core hours of the business, Monday to Thursday 9.00-5.30 pm and Fridays 9.00-5.00pm. The location is currently home-based working; however, you will be required to complete some home/community visits in Sandwell following a dynamic risk assessment of the needs of the visit. Pay rate is £15.36 – £17.52 per hour.

The service aims to improve the outcomes of residents in Sandwell by :-

• Enabling/facilitating timely discharges from hospital
• Monitoring and facilitating timely discharges from intermediate care provisions
• Offering alternatives to hospital admissions
• Supporting people to return to independent living.

• Complete work in a timely manner/& adhere to deadline, timescales, whilst following policies/procedures
• Work as part of a team and deliver an enhanced customer experience that promotes the wellbeing of vulnerable adults to enable them to live in their own homes independently for longer,
• Supporting them in retaining and regaining their skills and confidence.
