An exciting opportunity has arisen in the Access Team in Perth & Kinross Adult Social Work & Social Care Service for 4 full time qualified Social Workers. The successful candidates will be required to work across the Hospital discharge Team, enhanced care home support, Transitions and Access Team. This may include working evenings/ weekends to support the Hospital Discharge Team and Out of Hours Service. Although the posts will be based within the Access Team, you may be required to work anywhere within the Adult Social Work and Social Care Service.
In this role, you will provide a professional service to a range of service users, assess and make recommendations and respond appropriately. You will work with a range of partner agencies and services in various settings to support service users.
•Manage or support frontline service delivery or specialist projects.
•Contribute to the operational requirements for service users to meet legislative requirements and contribute to the overall efficient delivery of the Council’s Social Care Services.
•Ensure the implementation of policy and relevant operational guidance to support service delivery.
•Promote and deliver high standards of services within localities across areas of responsibility and aim to deliver positive outcomes.
•Liaise and work with other agencies and partners to ensure service users are supported and to share knowledge and best practice.
•Demonstrate ability to assess individual needs.
•Manage the planning and the delivery of a variety of caseloads or projects involving the specialist knowledge or assessment.
•Provide advice and guidance to staff and partners supporting the relevant service user group including interpretation of legislation and national initiatives.
•Plan and co-ordinate programmes, initiatives and activities across a variety of services in line with the National Outcomes.
•Engage and work with a range of partners, service users and communities to support collaborative and partnership working and develop outcome focused plans.
•Knowledge of a range of public, independent and voluntary sectors and the social contexts of these fields.
•Ability to prioritise constant and conflicting demands.
•Possess a high level of communication skills to advise and inform others of outcomes and produce a variety of reports, presentations and statistical information.
•Take responsibility for yourself, customers, contractors and others and enable compliance with all relevant Health and Safety legislation.
•Promote and act as a role model for the Council’s values and commitment to equality and diversity.
•Promote positive values and anti-discriminatory, non-judgmental practice and treat colleagues and customers with dignity and respect in line with the Employee Code of Conduct and SSSC Codes of Conduct.
•Manage and/or assess risk in relation to service delivery and contribute to the corporate risk management framework.
You must hold a recognised Social Work Qualification (Degree in Social Work, DIPSW, CQSW, CSS or other relevant Social Work qualification) and be registered as a Social Worker with the Scottish Social Services Council (SSSC) or within 6 months of commencing employment and will be further responsible for maintaining your annual membership. A driving licence is essential for this post.
This post may be suitable for candidates who have recently completed their Social Work Degree but have not yet graduated. In this circumstance, a Social Work Assistant contract will be offered until you receive your qualifying certificate and are eligible to register with the SSSC as a Social Worker.
