Your new role

As a key member of the multi-disciplinary team, you will provide an Intermediate Care Occupational Therapy Service for physically disabled people and people with multiple and complex disabilities (including those with mental health problems).

• You will primarily visit patients at home or in bed-based Intermediate Care to provide OT assessments of their functional abilities in relation to daily living activities.
• Conduct Community Care Assessments and collaborate with colleagues in Care Planning to deliver tailored support plans.
• Help patients and/or their carers achieve their recovery goals by advising and supporting them.
• Communicate with residential establishment employees about the rehabilitation needs of physically challenged people in their care.
• Recommend and arrange for the provision of equipment and adaptations to increase or maintain people’s independence and/or to support their carers.
• Train individuals how to use equipment, adaptations, and alternative ways safely in order to increase or retain independence and/or aid carers.

What you’ll need to succeed

• Understanding of the underlying legislation governing the provision of services to disabled individuals
• Knowledge of the effects of environmental and social barriers on physically disabled individuals
• Knowledge of safe manual handling procedures
• Continuous professional development knowledge for allied health professionals
• Ability to carry out accurate occupational therapy assessments
• Ability to problem solve with service users IT skills e.g. use of e-mail, word processing, and client database
• Effective rehabilitation skills
• Experience assessing clients and providing recommendations to service users/caregivers in a community context
• Working experience in a multidisciplinary team setting
• Working experience as a qualified occupational therapist in rehabilitation
