Locality Worker (Permanent and Temporary) (15 Posts) 35 and 17.5 hours
NLC7 £24,108 – £26,764 (Pro Rata)
Based in one of our locality disability teams, you will be a member of a team delivering high quality support/care packages to adults with disabilities, using person centred approaches in individual or small group settings. You will also be required to facilitate meaningful opportunities in mainstream community environments by accessing a range of activities which encompass social, leisure, lifelong learning and employment pursuits. In addition, you will lead on and contribute to the assessment, implementation, development and monitoring of individual support plans ensuring they continue to meet the needs of service users, including arrangements for measuring outcomes for and with individuals.
Educated to SVQ Level 2 in Social Care with a willingness to work towards SVQ level 3 within a defined timescale is essential. In addition, you will have experience of working with people with disabilities and of providing high quality personal care for individuals and you should also have the ability to complete / contribute to assessments and care planning.
You will require to become a Protecting Vulnerable Groups (PVG) Scheme member prior to commencing in this role.

Work Pattern: Working flexibly 5 over 7 days and evenings and weekends as required.
Temporary posts will be reviewed on a thirteen week basis.
