This is a Local Area Co-ordinator role, which involves working alongside a range of health and social care partners to support people affected by mental and physical ill health to access support and activities within their local communities to enable people to access the right support at the right time. The postholder(s) will support the setup of new initiatives within the Health and Social Care Partnership and NHS Fife and contribute to project development including support and referral pathways.
The postholder(s) will be required to work 36 hours per week Monday to Friday, with the requirement to travel throughout Fife, whilst overseeing a team of Community Connectors.
The post holder will be educated to SCQV level 9, which includes a degree or equivalent experience of working within a multi-agency and community setting.
The post holder will have experience of working within a community development or community care environment, with the ability to manage a staff team, which will work alongside individuals across Fife’s communities. The postholder requires strong verbal and written communication skills with a proactive and person-centred approach to innovative service delivery, collaboration with a range of partners and positive team building to deliver the best outcomes for and with people.
Before confirming appointment, the postholder will be required to obtain Protection of Vulnerable Groups (PVG) scheme membership through Disclosure Scotland and become a member of the relevant PVG scheme
