Job Description

We have an exciting opportunity for an Intermediate Care Worker in our fully integrated health and social care Intermediate care service at the Bellfield Centre. We are looking for an experienced, motivated and person-centred worker who can demonstrate strong values and a desire to work in partnership to improve outcomes for people using the service.

This is a 35 hour post with one hours contractual overtime each week.

Requirements
