As a care provider we have an essential role in keeping individuals with care needs safe from COVID – 19 and throughout winter, it will be essential that we continue to provide care at home to prevent avoidable admissions and support safe and timely discharge from hospitals. We are enhancing our current Home Assessment Recovery Team to allow us to be more adaptable and responsive to provide the support required to enable people to stay at home. This role will involve coordinating care packages and supporting a team of skilled care staff to deliver excellent support to people at home. If you have exceptional organisational skills and the ability to prioritise and support the delivery of a fast paced service, we need you in our team. You will be working across an office based location and at home. It is mainly office hours but there may be occasions when you will be required to work at weekends and evenings.
It is essential that you have excellent listening, oral and written communication skills as well as the ability to lead and work as part of a team. You must also have the ability to maintain confidentiality, work co-operatively with other professionals, families and carers and be able to work on your own initiative. This role will be ideal for you if you are enthusiastic, motivated and able to cope with demanding work. We expect you to be reliable and have a flexible approach together with a positive, responsible and caring attitude. This will be a demanding post that will allow you to demonstrate your flexibility and showcase your proactive approach that can respond to the changing needs of individuals.
